Jane's Personal Health Action Plan

Jane has been trying to lose weight all year. After learning more about eating healthfully, Jane has successfully shed 20 pounds. She still has more to lose and is worried that the holidays will get her off her healthy eating habits. Here is Jane's plan:

Personal Health Action Plan

My goal: To lose weight and/or maintain my weight through the holidays.
Steps I will take:        What    --    When    --     How often
  1. Bring a healthy dish I prepare to my family party.
  2. Have a light, healthy meal before going to a party.
  3. Allow myself to eat my favorite cookies my sister makes. I will eat a light lunch on that day.
  4. I will skip the cocktails at all the parties so that I can spend the calories on sweets instead.
  5. Follow my regular eating plan on days there are no parties.
Benefits I will get from meeting this goal:
  • Feel in control of my eating
  • My clothes won't get tight!!
  • Continue to lose weight
  • Will still enjoy the holiday foods
Barriers that might get in my way:
  • My sister will insist I eat more cake, candy, etc.
  • People will try to send goodies home with me
  • My husband likes for me to make fudge
How I will overcome these barriers:
  • Plan what I will say to my sister when she offers more cake, accept that she may not understand
  • Tell people we won't be home and the food will be wasted
  • Make a small amount of fudge for my husband (he can hide it somewhere)
Who can help me with my goal? How?
  • My friend Jean whom I volunteer with is also trying to lose weight.
  • My husband - I can tell him how important this is to me and he will help.
  • I will check to see if OASIS is offering any classes that may help.
Date I will check my progress:
  • I will keep track of the foods I'm eating in a notebook.
  • I will weigh myself weekly and keep track of my weight loss.
How I will reward myself for meeting my goal:
  • I will buy myself some new clothes or a book - something I wouldn't normally treat myself to.
